2010 Summer Registration & Medical forms |

Please complete both the Registration and Medical Information forms, typing in all fields. Print the form off and forward it onto the address on file or turn into Club Registrar. Registration fee: $35 tryout fee for 2010 summer ($25 of which will go toward $135 Club fees. Club fees for 2010 season did not change from the 2009 season fees) Click here for Club fee details. Team fees are in addition to club fees. NOTE: Completion of the form does not guarantee placement.
